Total Diabetes Care (TDC) is a full service diabetes education center located in the Greater Los Angeles Area. TDC focuses on preventative health and wellness services for all populations with a special emphasis on the underserved populations in Los Angeles. As part of their health and wellness program, TDC utilizes the services of a Certified Diabetes Educator (CDE), registered nurse, registered dietician and a local endocrinologist consultant. TDC has formed linkages with the medical and pharmacy community and receives referrals and support from the Drew Medical Society, Golden State Medical Association, Association of Black Women Physicians, Primary Doctors Medical Group, Central Los Angeles Pharmacist Association, and the African American Chapter of the American Diabetes Association.

Key Messages:

TDC believes that through education people with diabetes can improve their overall health and delay / prevent the short and long term complications of diabetes. (i.e. blindness, amputation, kidney failure, cardiovascular disease).

TDC advocates the use of intensified diabetes self-management through the appropriate use of oral medications and intensified insulin therapy. TDC will provide consultative educational services to clinicians and patients to maximize adherence to individualized therapeutic regimens.

TDC believes that people with diabetes require ongoing educational programming and diabetes screening of family members to provide early detection and treatment for type 2 diabetes. To accomplish this goal, TDC will provide diabetes support groups in both English and Spanish and Diabetes Screening Programs targeting high-risk patient populations.

TDC believes that people with diabetes require access to a full range of information regarding the diagnosis and management of type 2 diabetes. In order to accomplish this TDC will establish "state of the art" interactive computer services and Internet web access to "Diabetes Watch" in order to provide comprehensive on-line educational services to both patients and providers.
