Provided through interactive group sessions and individualized appointments. Patients are provided with self-management techniques and the knowledge necessary to achieve optimum glycemic control to minimize long -term complications. The curriculum of the education program is in compliance with the National Standards for Self-Management Training and current HCFA guidelines for Medicare Reimbursement.

Program Structure:

Diabetes education is provided either as a Group Class or through Individual Appointments. The group class will be provided in English and/or Spanish. The Group classes will be a total of ten (10) hours. Six (6) hours with a diabetes educator and four (4) hours with a registered dietician. Individual sessions (1-hour) by appointment. Physician referral required for insurance billing.

The cost of the ten (10) hour program is based on the current Medicare reimbursement for Diabetes Education. The cost of the ten (10) hour program is $300.00 ($30.00 per hour). Payment will be secured from the patient's medical insurance carrier or will be obtained directly from the patient. Individual session will be one hour in length. The cost of the individual session is $55 per hour. CASH DISCOUNTS AVAILABLE
